We understand your concern for the safety and protection of your children on Sunday mornings and we have installed a computerized check-in system.
 
Please click here for a form that you can print, fill out and return to Ms. Kenna, OR click here to email Ms. Kenna the following information:
 
  1. your child's name
  2. birth date
  3. age
  4. grade and school attending (if applicable)
  5. parent/guardian name(s)
  6. address
  7. city, state
  8. phone number
  9. e-mail address
  10. permission to send your child e-mail
  11. name(s) of any other adult who is authorized to pick your child up from class
